L.E.A.P.’s new Community Care Coordinator Jordyn Avery will be reaching out

L.E.A.P. would like to announce Jordyn Avery as our new Community Care Coordinator!

Jordyn will be reaching out to all those who received or utilized any of L.E.A.P.’s  programs and services to see how you and your family have been managing this past year, how you are doing now, and find out what else L.E.A.P. can do to assist members of the community.

Jordyn is new to this position, but not new to L.E.A.P.  Prior to becoming the Community Care Coordinator, she had been working as a Home Visitor for the Cambridge Center of L.E.A.P. Head Start since June 2019.  She will now be working out of the L.E.A.P. Career and Family Services suite at 383 Broadway in Fort Edward.  Jordyn lives in Greenwich, New York, has two cats, and enjoys hiking and doing yoga in her free time.

As the Community Care Coordinator, Jordyn can assist with finding resources and support for you or your family member (food, financial, emergency assistance, budgeting, mental health, healthcare, education, training, employment, transportation, etc.) toward the goal of self-sufficiency and overall well-being in these difficult times.

According to Jordyn, she is most excited about her new role to create a long lasting impact on our community.
